CAS Premier Health - General Information

To allow Dr. Stevens to spend more time with patients and to provide more one-on-one, dedicated services, Dr. Stevens has developed a retainer-style practice called CAS Premier Health. This type of practice is becoming more widespread and has also been called “patient-centered healthcare,” “boutique medicine,” “concierge medicine,” “executive health program,” “personalized healthcare,” and “platinum practice.”

CAS Premier Health will be integrated with Triangle Family Care and will start on January 1, 2010. For an annual retainer fee, those patients who become members of CAS Premier Health will be entitled to the following non-insurance-covered services:

Better Access to Dr. Stevens

24/7 access to Dr. Stevens via cell phone and/or email
Same or next-day appointments with him
Longer time available for each office visit
”After-hours” office visits including evenings, weekends, and holidays, if necessary
House calls, if needed
Visits to you at your office, if needed
Unlimited number of office visits during the year

More Personal Assistance Related to your Healthcare

Accompaniment by Dr. Stevens or a staff member to specialist office visits, if needed
Travel assistance, when available, to specialists; such as for a colonoscopy or outpatient procedure
Access to your medical records via portable electronic storage device (i.e. disc or jump drive) when electronic medical records are implemented at Triangle Family Care

Inpatient Care by Dr. Stevens rather than a Hospitalist, when needed

Emergency department (emergency room) visits
Coordination of inpatient care (Dr. Stevens will have admitting privileges at Wake Med Cary and Rex by 1/1/10)

Additional Benefits

Discounted fees for those patients who do not have any health insurance
Included membership for all children less than 18 years of age (or 23 years of age, if a full-time student) when both parents or two adults in the same family are fully paid members
Annual, comprehensive physical exam and wellness plan. (If your health insurance company pays for an annual physical, you will be reimbursed the amount that your company pays for this.)

The annual retainer fee for the above services effective January 1, 2010 will be $1,080.00 (less than a cup of Starbuck’s coffee per day!), if paid at one time before January 31, 2010 or $1,200.00 per year if one half ($600.00) is paid before January 31, 2010 and the other half ($600.00) by July 1, 2010.

Membership in CAS Premier Health will be limited to 600 members. Each year CAS Premier Health will offer 50 free memberships to patients who cannot afford the annual membership fee. These free memberships will be decided by a drawing to be held the first day of December for the following membership year. If you are interested in a free membership, please complete a request card before December 1st and give to any of the receptionists at Triangle Family Care.

For those patients who opt not to become members of CAS Premier Health, Dr. Stevens will still continue to supervise the excellent healthcare that is provided to patients of Triangle Family Care by our Family Nurse Practitioners, Crystal Moran, FNP and Kellie Furin, FNP. Also, Dr. Stevens will see any patient, whether a member of CAS Premier Health or not, for an annual physical.
